Facility Evaluation Report

Licensing Program Analyst (LPA) conducted an unannounced case management to obtained documents pertaining to complaint control no.: 31-AS-20220728112027 regarding Resident #1 (R1). LPA met with staff James Olalia and explained the reason for the visit. LPA called Administrator Frances Martir and designated Mr. Olalia to sign the report.

LPA conducted physical plant tour at 9:15 AM and interviewed staff and Administrator. LPA attempted to obtain documents regarding R1 but was told and observed that R1's record is longer at the facility. LPA's interview with the Administrator revealed that the Administrator is not aware of where the documents are but will try to find it and email it to the LPA.
