The inspector’s account

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Plan of Correction (POC) visit on 07/31/2026. LPA was greeted by Administrator Joel Alonzo and explained the purpose of this visit.

On 07/03/2026, LPA Ramirez conducted an unannounced Annual Continuation Inspection and cited five (5) deficiencies that were observed during the inspection. One of the deficiencies issued was for violation of California Code of Regulations, Title 22, Division 6, Chapter 6, Acceptance and Retention Limitations - 85068.4(f): The Department may require the licensee to comply with various regulations applicable to RCFEs if the Department determines that compliance with any such specific regulations is necessary to protect the health and safety of clients 60 years of age or older. Such regulations may include, but not be limited to, those pertaining to the training of staff members who assist clients with personal activities of daily living; the regular observation of clients for changes in physical, mental, emotional, and social functioning; and the notification of the client's physician and responsible person and/or authorized representative, if any, of documented changes. This deficiency had a Plan of Correction (POC) due date of 07/17/2026 and was agreed upon and signed by LPA and Administrator Alonzo.

On 07/29/2026, LPA Ramirez received Proof of Correction form (LIC 9098) and proof of staff receiving training on several subjects regarding clients over the age of 60, via email. The Proof of Correction Form indicated the correction was completed on 07/27/2026.
