The inspector’s account

On 01/28/2026, Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Incident visit regarding the unexpected death of client#1 (C1) on 01/25/2026. LPA Ramirez was greeted by 01/28/2026 and explained the purpose of the visit. LPA Ramirez toured facility and conducted wellness inspection.

On 01/26/2026, LPA Ramirez received Death Report (LIC 624A) which revealed on 01/25/2026 at 9:50 pm, C1’s roommate alerted staff that there was something C1. Staff immediately went to go check on C1 in their room and found C1 sitting in their chair unresponsive. Staff immediately called 911 and began CPR until emergency services arrived. C1 was pronounced deceased at 10:35pm. Norwalk Sheriff’s Department were notified and provided the facility with an incident report regarding C1’s death. C1’s family was notified and made arrangements to have C1’s body taken to a mortuary.

On 01/28/2026, LPA Ramirez conducted a wellness inspection and did not observe any immediate health and safety violations. LPA requested and obtained C1’s recent physician’s report, emergency contact information, admission agreement and medication administration record (MAR) for the months of December 2025 & January 2026, and needs and services plan. Interview with Administrator Joel Alonzo revealed that the Norwalk Sheriff’s Department told him that detectives and the coroner would not be arriving due to no signs of foul play. Administrator Joel Alonzo agreed to send LPA Ramirez a copy of C1’s death certificate as soon as it becomes available.

No deficiencies were cited during this visit. A copy of this report was provided.
