Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted the required annual inspection. LPA arrived unannounced and met with the Administrator. The reason for the visit was explained. The facility is licensed for (6) adults, ages 18 - 59, ambulatory only. The facility is vendorized by the Eastern Los Angeles Regional Center. There are currently 3 clients residing at the home.

LPA inspected the facility using the Compliance and Regulatory Enforcement (CARE) tools.

The facility consists of 2 client bedrooms, 2 staff rooms, 3 bathrooms (1 for client, 2 are in the staff room), living room, dining room, family room, kitchen, and attached garage. The bedrooms have appropriate furniture and storage space. There are extra linens and hygiene supplies. Facility has smoke and carbon monoxide detectors. Knives and cleaning solutions are locked. The hot water temperature was measured between the required range of 105-120 degrees F. Sufficient food supplies of 2 day perishable and a week of non-perishable items observed. LPA reviewed records and medications for 3 clients. The clients files have the required documents and medications are being administered as prescribed. Medications are centrally locked. LPA also reviewed 2 personnel records. Administrator (Aida Ramirez) certificate is not current. The Administrator Certification Bureau website does not show that the administrator has an active certificate or pending renewal. The HIV & TB certificate was issued on 11/17/25. Facility staff have current CPR & First Aid certificates. The facility has an emergency disaster plan and is conducting quarterly drills.

A deficiency is being issued today. An exit interview was held and a copy of this report, along with appeal rights, was given to Ms. Ramirez.
