Facility Evaluation Report

Licensing Program Analyst (LPA) Galarza conducted an unannounced Required- 1 year visit. The purpose of the visit was explained to Administrator Guadalupe Sahagun. The facility is licensed as a level 2 Adult Residential Facility serving developmentally disabled residents 18-59 years by Eastern Los Angeles Regional Center.

Infection Control: The Infection Control Plan includes Environmental cleaning and disinfection activities.

Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood that is licensed for four (4) ambulatory residents. It consists of 6 bedrooms [3 designated for residents and 3 for family use], living room, family room, dining room, kitchen, laundry room, utility room, office, covered backyard deck area, and gated self-latching swimming pool. The office room has a bed; licensee uses the room as a family guest bedroom. Smoke and carbon monoxide detectors were tested and are operational. The facility has one (1) fire extinguisher and a fire pull alarm. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. The facility has a 1st Aid Kit and Manual. Cleaning supplies, knives, and toxic substances are inaccessible to residents. Exit doors are free of any obstruction. There is a pool in the backyard that is gated.

Operational Requirements: Fire clearance is approved for four (4) ambulatory residents. Care and supervision to meet the clients needs was observed. No special equipment and supplies are used by residents. The Surety Bond is current with an effective date of 6/23/2025.

Staffing: A total of 2 staff members provide care and supervision to the clients.
