Facility Evaluation Report
On 05/29/24 at 12:50 pm, Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Administrator Leilani Le and the purpose of today’s visit was explained. The facility is a short term crisis program licensed to operate for (6) ambulatory adults ages 18 through 59. Currently, the home has (2) clients. None the clients have Restricted Health Care Conditions, and none are utilizing postural supports or protective devices.
The facility is a two story family home located in a residential neighborhood. The property consists of the following: DOWNSTAIRS * Living room , dining area , Den, kitchen, laundry room, 1 common bathroom, Staff Office including staff bathroom. UPSTAIRS * 3 client bedrooms and 2 bathrooms. There is a detached storage room in the back of the home. There is an outdoor shaded area.
LPA conducted a records review of (2) client records, (2) staff records, and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. All clients attend Clearview Treatment Day Program, where they attend from 10:00am to 7:00pm All meals and medications are given there.
