Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Yee conducted an unannounced case management visit due to deficienciesd observed during today's visit. LPA Yee was let into the home by Emani Snaer, Staff. Zohrab Zargery, Administrator was contacted by telephone and he arrived at 1:35pm to conduct the visit. The reason for today's visit was explained.

During this visit the following was observed:

Emani Snaer, staff was hired on 8/5/25 and was observed working at the home during this visit. Per verification with the office, the staff has a criminal record clearance but the criminal record clearance was not transferred to this home.

Staff from Pyles and Associates who provide 1:1 care to Resident #1 from 9am - 9pm from Sunday through Saturday have no evidence of a criminal record clearance for the three staff that is currently providing the care and are not associated to the facility. Per information received from the Administrator, and verified with the staffing agency, the following are the 3 staff working with Resident #1 - Aysha Shah, Koebe Oliver and Rocio Torres. They have being providing services to Resident #1 since the resident was admitted to the facility on 6/16/25.

Deficiencies were cited under California Code of Regulation, Title 22, Division 6, Chapter 8. Civil Penalties of $2000 was assessed.

Exit interview was conducted, Appeals Rights discussed and a copy was provided.
