Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the required annual inspection on 7/29/25. LPA arrived unannounced and met with the administrator, Josefina Teodoro. The reason for the visit was explained. The facility is licensed for (6) developmentally disabled adults, ages 18 - 59, ambulatory only, and is vendorized by the San Gabriel/Pomona Regional Center. There are currently (5) clients residing at the facility.
LPA inspected the facility using the Compliance and Regulatory Enforcement (CARE) tools.
The facility consists of 3 client bedrooms, 1 staff room with a private bathroom, 1 communal bathroom, a living room, dining room, family room, and kitchen. The laundry machines are in the garage. The client's bedrooms have the appropriate furniture and storage space. There are extra linens and hygiene supplies. The facility has operable smoke detectors and a carbon monoxide detector. Knives and cleaning solutions are locked. The hot water temperature was measured between the required range of 105-120 degrees F. Sufficient food supplies of 2-day perishables and a week of non-perishable items are observed.
Staff are providing care and supervision to meet the clients' needs. Staff are all fingerprint cleared and associated to the facility. LPA reviewed records for 3 staff and 5 Clients. Administrator's (Josefina Teodoro) certificate expires on 12/15/26. Facility staff have current CPR & First Aid certificates. Staff do not use any manual restraints on clients. Client files have to required documents. There are no clients with restricted or prohibited health conditions. Medications are centrally locked and inaccessible to clients. LPA reviewed medications for all 5 clients, and they are being administered as prescribed. LPA issued technical violations and an advisory note for today’s visit.
No citations were issued today. An exit interview was held, and a copy of this report and technical notes were given to the administrator.
