On 10/22/2025, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Annual Inspection. LPA met with Licensee/Administrator, Joseph Sol explained the purpose of the visit and was allowed entry to the facility.
A facility tour was conducted and records were reviewed.
Due to insufficient time, the Annual Inspection was not completed.
An exit interview was conducted and a copy of this reports was left with the Licensee/Administrator, Joseph Sol.
