Licensing Program Analyst (LPA) Jose Tan conducted an unannounced case management visit at this facility to issue deficiency. LPA met with Assistant Administrator Glacie San Juan and explained the reason for the visit.
During physical plant tour for the Required annual visit for a sister facility in the same compound (A Timia Oasis) today, LPA discovered that this facility has seven (7) residents currently living in the house.
Citation issued. Civil penalty assessed. Appeal rights discussed and given.
Exit interview conducted. Copy of this report issued.
