On 10/03/2024, LPA de la Cerra attempted to do an unannounced case management visit at this facility.

LPA's purpose of the visit was to discuss the details pertaining the eviction notice sent by the property owner of 8045 Grove street to the Unique Group Home licensee/administrator.

LPA rang the doorbell at the facility at 2:12pm and at 2:30pm, for both times, there was no answer.

LPA while still in the area close to the facility, proceeded to contact both the property owner and the licensee.

At 2:48pm LPA contacted property owner's number but the contact number was no longer valid.

At 2:52pm LPA contacted licensee for Unique Group Home.

Licensee for Unique Group Home answered. The phone conversation between LPA and licensee was approximately 20 minutes long. At the beginning of the phone conversation, as soon as LPA informed Licensee the reason for the phone call, the licensee immediately expressed a lot of dissatisfaction during the phone conversation. Licensee mainly dominated the phone conversation, so LPA was unable to ask licensee whether the licensee was currently inside the facility.

Licensee did inform LPA that the eviction matter will be addressed in court between the licensee and the property owner. Licensee also stated to LPA that he refuses to vacate the property.

LPA concluded the phone conversation with the licensee due to the fact that the licensee continued to be in a dissatisfied state. Thereafter, LPA was advised by LPM to leave the area.
