Facility Evaluation Report

Licensing Program Analyst (LPA) Teresa Camara conducted an unannounced case management visit regarding an elopement incident which took place on 07/18/2022. LPA met with administrator Kathy Shadeko-Adedigba and explained the reason for the visit.

On 07/18/2022 at approximately 1:45 p.m. Resident 1 (R1) was in the backyard relaxing when R1 got up and left the facility through the side gate. Staff 1 (S1) followed R1 but lost R1 at an intersection. S1 contacted the administrator immediately and they began a search of the neighborhood. The Simi Valley Police Department was also called. At approximately 2:49 p.m. the police notified the administrator that R1 was located at a residence approximately 1.5 miles away. R1 was knocking on the door of the residence because R1 thought that was their brother's home.

LPA interviewed the administrator at 2:42 p.m. She indicated R1 was admitted to the facility on 4/16/2019. R1 has one on one (1:1) staff 24 hours a day. R1's 1:1 staff that day, staff 1 (S1), was not working during LPA's visit. On the date of the elopement there were two staff at the facility and 3 residents. The other two residents do not require 1:1 staffing. The facility AWOL protocol is to follow R1 at a distance and not run after R1 because R1 will bolt into the street. R1 has a history of elopements and the administrator has notified Regional Center that this is not appropriate placement for R1. The administrator gave R1 and Regional Center a 30 day notice on 02/28/2020 but Regional Center has not been able to find other appropriate placement for R1 at this time; they are still working on it.

LPA reviewed R1's file and AWOL protocol was observed. It was also noted in R1's Individual Program Plan (IPP) that R1 has a history of AWOLs, property damage, aggression and disruptive behaviors.

No deficiency cited at this time. Exit interview conducted and a copy of the report was emailed to administrator.
