On 08/05/2026, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced case management - incident visit regarding Client#1's death report the Department received on 07/22/26 12:54 PM. LPA met with Administrator Stephanie Brown and Staff.
During the visit, the Department obtained Client #1's record and interviewed Staff #2.
Due to insufficient time, a follow up visit is required.
An exit interview was conducted and a copy of this report was provided to the Staff Terry Santos.
