The inspector’s account

Licensing Program Analyst (LPA) Gabriela Castro conducted the annual inspection and met with Co- Administrator Danelle Loboda . The purpose of the visit was explained.

The facility is a single-story property in a residential area with seven buildings. It serves 38 ambulatory clients, all of whom receive case management through San Gabriel Pomona Regional Center.

During the visit, LPA used CARE tools and observed the following:

Smoke and carbon monoxide detectors were working properly. Fire extinguishers were accessible, fully charged, and located in each building, including the kitchen and laundry room. Knives, cleaning supplies, and disinfectants were locked and not accessible to clients. Hot water temperatures ranged from 103.6°F to 113.8°F across all buildings. The outside carport area had old mattresses and trash debris that need to be removed.

Medications were centrally stored and locked in a medication cart. Eight client medications and MARs were reviewed. It was noted that MARs were not completed accurately and PRN medications were missing for all eight (8)clients.

Due to time constraint LPA will return a later date and time for a continuation visit.

An exit interview was conducted with Danelle Loboda, Administrator During the inspection, deficiencies were observed and cited on the attached LIC 809D/809C in accordance with Title 22, Division 6 regulations. A copy of this report, LIC 809D/809C, and appeal rights were provided via email.
