Complaint Investigation Report
Allegation: Personal Rights/ Staff sexually assaulted client in care. It has been alleged that staff sexually assaulted a client in care. Staff interviews revealed that staff do not assault (including sexually) clients in care. Interviewed staff indicated they have not witnessed nor received any complaints/concerns regarding staff mistreating, assaulting and/or isolating clients. Interviewed staff indicated they are trained in Mandated Reporting, Zero Tolerance and Client Rights. Client interviews revealed that staff do not assault (including sexually) clients in care. Interviewed clients indicated that they have not witnessed nor heard anyone complaining/having concerns regarding staff mistreating, assaulting and/or isolating clients. Interviewed clients indicated they feel safe and comfortable residing at this facility. Interviewed clients indicated that they do not have any concerns. Per interviews conducted, Pomona Police Department visited this facility on 04/20/25 to conduct a health and safety visit pertaining to this allegation. Per interviews conducted, Pomona Police Department did not open a case/investigation and only conducted a “call for service” visit as there were no health and safety concerns. Special Incident Reports for the 04/20/25 alleged incident were also sent by the Facility Administrator to Community Care Licensing and San Gabriel Pomona Regional Center. Interviews do not corroborate this allegation.
Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
