Complaint Investigation Report

During interviews with the clients, none of them corroborated the allegation. During IB investigator Seng's interview with C1, they denied that they ever had a sexual relationship with S5. During interviews with the other clients of the facility, none of them stated that they directly witnessed S5 being engaged in a romantic or sexual relationship with C1. During interviews with the staff, none of them corroborated the allegation. One staff interviewed stated that S5 was terminated due to excessive absenteeism, and only found out about the allegation after the fact, to which C1 stated that this never occurred. Other staff stated that S5 and C1 denied the allegation as well, and never confirmed if they were ever in a relationship. LPA Erik Zaragoza attempted to contact S5 but did not get in contact with them.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted. A copy of this report, along with the appeal rights, was provided.
