Complaint Investigation Report

Interviews with clients revealed that 8 out of 10 residents could not corroborate with the allegation. Interview with C1 revealed that C1 is not sure if staff did address the issue, but C1 has not heard C4 use the racial slur since the day that it was reported to staff. C4 denied using racial slurs.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
