Complaint Investigation Report

Social Security provided the September check for C1 to the facility on 9/3/22. Interviews show that the facility will return SSI payments to the Social Security Administration (SSA) office whenever clients leave the facility but SSA still sends payments to the facility. LPA reviewed documents showing that the facility returned a total of 1385.77 to the Social Security Administration (SSA) on 11/29/22 stating it was from C1's September income. Bank statement reviewed shows the check was cashed by SSA on 12/1/22. LPA was not provided with proof that C1 has not received the refund as C1 had not verified with SSA office. Based on LPA's interviews, observations, and documents reviewed; although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
