The inspector’s account

Investigation revealed the following: Regarding allegation, Staff are not ensuring that resident's teeth are brushed , it is alleged that abuse and neglect have been witnessed at facility and that client's teeth are always full of plaque and are never brushed. Allegedly all clients are always in their rooms locked away and staff will say that they are asleep when it is 4pm. Clients are timid around staff and there is a client that picks at their skin and runs to room as if afraid of facility staff. Interviews conducted with 2 out of 3 clients revealed that staff ensure that they are brushing their teeth twice a day, in the morning and at before bed. They also stated that they are taken to the dentist for check ups, they are not locked away in their rooms by staff and stated that facility staff are nice and take care of them. 1 client was not interviewed as they are non-verbal. Interviews conducted with 3 staff revealed that staff ensure that all clients brush their teeth every day in the morning and before bed. They stated that clients are taken to their dentists twice a year for their check ups, clients are not locked away in their rooms at 4 pm and stated that a client will be in their room at time if they chose to be. Staff stated that clients are very comfortable with staff and denied that any client is timid or scared around any facility staff. Staff also denied that there are any clients that pick at their skin due to being afraid of staff. LPA observed 3 facility clients during visit and observed clients to look clean and interacting with staff in the living room. LPA observed that there was enough hygiene supplies for each facility client. LPA additionally observed that clients are taken regularly to their dentist for follow ups. Based on LPA observations, LPA review of client files, and statements gathered from interviews conducted with staff and clients there was not enough supportive evidence to concur with the reported allegation.

For allegation, Staff left resident soaked in urine , it is alleged that a client was observed to be soaked in urine, to the point where it seemed they had used the bathroom on themselves more than once. Interviews conducted with staff revealed that clients are never left soaked in urine. They stated that if a client happens to have an accident, the client is immediately changed and showered and their linens are changed out and washed. Staff stated that client linens are washed twice a week and immediately if any of them have an accident. 2 out of 3 clients stated that staff clean their rooms everyday. They stated that their beds are clean all the time. 2 clients stated that staff are nice and take good care of them, they take a shower everyday and they are not left in soaked linens. LPA toured client bedrooms and observed all client beds to be clean and have clean linens. LPA did not smell an odor that can be described as urine in any client room. 1 client was not interviewed as they are non-verbal. Based on LPA observations, and statements gathered from interviews conducted with staff and clients there was not enough supportive evidence to concur with the reported allegation.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED .
