Facility Evaluation Report
Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Damion Jackson and explained the reason for the visit. Claudia Cortez arrived at approximately 9:20 A.M..
This program is approved to serve (55) ambulatory and (10) non-ambulatory clients ages 18 through 59. This program consists of a lobby, a conference room, (3) offices, (6) classrooms, (1) activity room, (1) community room, (2) participant restrooms, a kitchen, a copy room, (1) staff restroom. Clients from this program receive case management from a Regional Center (Frank D. Lanterman, Eastern Los Angeles and San Gabriel Pomona).
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Facility has an Infection Control Plan in place.
Physical Plant & Environment Safety: Smoke alarms and carbon monoxide observed. Fire extinguishers observed throughout this facility (last serviced on 05/21/25). Last drill was conducted on 11/06/25. Hot water temperature measured between 115.7* (women’s bathroom) and 115.3* (men’s bathroom). Water fountain with filtered water was observed.
Operational Requirements: Staff are adhering to operational requirements.
