Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management - Incident visit today regarding a special incident reported to the department by facility staff on 02/02/2026 involving Client #1 (C-1) and Staff #1 (S1). LPA met with Wesley Mair, Live-in Staff and explained the purpose of today’s visit. Segundino Gotladera, Administrator, arrived thereafter and continued assisting with the visit.

During this visit, LPA reviewed C-1 and S1’s files and obtained copies of relevant documentation including staff and client rosters, staff contact numbers and police report information. LPA toured the facility grounds and food supply and did not observe any health and safety concerns. During visit, five clients were attending Day Program and one client was home. Administrator will continue to keep LPA informed of any further developments pertaining to the incident.

An exit interview was conducted with Segundino Gotladera, Administrator, and a copy of this report was provided.
