The inspector’s account

Licensing Program Analyst (LPA) Alberto Lopez conducted an unannounced case management visit. LPA met with Staff Evelin Romero, and discussed the purpose of the visit. The purpose of the visit is to follow up on an incident report dated 11/12/2025 Administrator Marites Gacayan showed up a short time later and assisted with the visit .

According to the report, on 11/11/2025 around 8:00 am Client#1 woke up in bad mood. Access ride showed up for C1. Roommate of C1 – C2 went outside to verify who the ride was for. C1 pick up time was 8:40am and C2 was at 8:15am. The access driver stated he was there for both clients but C1 got angry and refused to go. C2 went back inside the home for 3 rd time and told C1 that access is not leaving without C1. C1 got angry and threw phone at C2 and started kicking and hitting C2. C2 fought back until staff intervened. Both clients sustained small scratches around the face area and C2 sustained small bruise on left index finger from hitting wall. Ice pack was applied to C2 finger. C2 apologized to C1. Both clients stated they feel safe at facility.

During the visit LPA reviewed staff and client rosters and interviewed Administrator and Clients #1 and #2.

Appropriate parties were notified of incident, and LPA did not observe health or safety concerns during visit.
