Facility Evaluation Report

License Program Analysts (LPA) Luis De Leon conducted an unannounced annual required visit. LPA met with Administrator Leah Avila. The purpose of today’s visit was explained. The facility is licensed to serve 6 ambulatory clients age range from 18-59 years old. Clients are serviced by Frank D. Lanterman Regional Center.

The LPA use the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. The visit consisted as follows:

The facility is in a residential area and is made up of a kitchen, a dining room, a living room, (4) bedrooms; of which (3) are shared client bedrooms, and (1) staff room, 1 full bathroom, 1 half bathroom, a laundry area, a garage, a porch with shaded sitting area, and a backyard area.

Client record review consisted of Admission Agreements, Identification and Emergency Info, Physicians Report, Consent Report, Individual Program Plan, and Personal Rights. Staff record review consisted of Personnel Report, Health Screening, Criminal Record Statements, Fingerprint Clearance, Training, First Aid and CPR.
