Facility Evaluation Report

Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced case management visit to this facility. Upon arriving at the facility, LPA met with Assistant Administrator / Alexander Solorio who assisted with the visit. The purpose of today's visit is to review the files of Resident 1 -5 (R1 - R5).

Due to time constraints, LPA had to terminate visit and will return on a later date to complete the visit.

An exit interview was conducted and a copy of this report was provided to the Assistant Administrator.
