Facility Evaluation Report

Licensing Program Analyst (LPA) Luis De Leon and Licensing Program Manager (LPM) Fernando Fierros conducted an unannounced case management visit. LPAs met with the Administrator Carolee Hazzard and explained the reason for the visit.

During today's visit, LPA reviewed clients records, and during client #1 (C1) record review, LPA noticed that C1's medical assessment was not present in C1's facility file. Administrator confirmed that C1's medical assessment was not present in C1's facility file and facility does not have a copy. A deficiency is noted in LIC-809D based California Code of Regulations, Title 22, Division 6 & Chapter 1.

Exit interview held and a copy of the reports LIC-809, LIC-809D, and appeal rights were provided and discussed with Administrator Carolee Hazzard.
