Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Case Management Visit – Plan of Correction (POC) to follow up on deficiencies cited during annual visit conducted on 4/23/2026. LPA met with Carolee Hazzard, Administrator, and the reason for the visit was explained.
During today’s visit, the following citations were corrected:
CCR Section 80075(b) – Health Related Services: assistance as needed with self-administration of prescription and nonprescription medications.
HSC Section 1565(a) – Other Provisions: facility has an Emergency and Disaster plan.
Exit interview was conducted with Carolee Hazzard, Administrator, and copies of the report and POC letters were provided.
