Complaint Investigation Report

Allegation: Facilities dining room chairs are not in usable condit io n. During this visit, LPA conducted a tour of the dining area and observed the chairs to be cushioned and comfortable (pictures taken). Interviewed Clients indicated they do not have any issues with the dining room chairs and that the chairs are comfortable and are not broken. Interviewed staff indicated there are no issues with the dining room chairs. Dining room chairs are in useable condition. LPA observation and interviews do not corroborate this allegation.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

Note; LPA was experiencing technical difficulties during this visit.
