Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced required annual visit. LPA met with Crystal Rose, Administrator, and explained the purpose of the visit. The facility is licensed to care for (6) developmentally disabled Adults, ages 18-59, of which (2) may be non-ambulatory. The facility is operating within the scope of its license. Clients in care receive services from Frank D. Lanterman Regional Center. There are currently (4) clients living in the home and (2) clients were home during the visit.
The facility is a 2-story home located in a residential neighborhood of Pasadena. The home consists of a living room, (4) client bedrooms, (2) full bathrooms, (2) offices, kitchen, pantry, dining room, front and backyard, patio, and a detached garage with a laundry area.
During the visit, LPA toured the facility's indoor and outdoor environment, reviewed (4) client and (4) staff records and conducted medication review. During today’s visit, LPA observed the following:
Facility was observed clean, inside and out. All walkways, passages, ramps and exits are clear and free of debris and obstructions. Living room and dining area have sufficient seating and furniture is in good repair. Kitchen was observed to be clean and sanitary for the storage and preparation of food. Kitchen appliances are clean and were operating at the time of visit. Cleaning supplies and toxins were observed locked in a cabinet under kitchen sink and inaccessible to clients. Knives/sharps were also properly locked in a kitchen drawer. Sufficient supply of (2) day perishable and (7) day non-perishable food was observed. Food is labeled and kept within expiration limits. Overflow refrigerator and freezer are kept in the garage and both contained additional food which was also observed to be properly stored and labeled. Weekly menus are posted in the kitchen.
