Facility Evaluation Report
Licensing Program Analyst (LPA) Sanjay Vaid conducted an unannounced visit at the facility for the purpose of conducting the required annual inspection. LPA utilized the Compliance and Regulatory Enforcement Tool to evaluate the facility. LPA Lopez met with Mary Jule (Direct Support Professional) who allowed entry, LPA explained the purpose of the visit. Administrator Monique Jordan arrived a short time later and assisted with the visit. The facility is licensed to serve 6 ambulatory developmentally disabled clients aged 18 to 59. Currently, there are six (6) clients in placement from Laternman Regional Center.
Infection Control: The facility staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has sufficient PPE supplies, has an Infection Control Plan and Mitigation Plan. Bathrooms have hand washing signs, soap and paper towels. Facility Administrator is adhering to infection control requirements.
Operational Requirements: Fire/ Earthquake drills are conducted every three months; the last fire/earthquake drill was conducted on 01/08/2025.
Physical Plant & Environment Safety : Facility is licensed for residents 18 to 59 years old. The facility has a fire clearance approved for six (6) ambulatory. There are six (6) developmentally disabled clients residing at this facility. Residents at this facility are receiving services from Frank D Lanterman Regional Center. LPA requested and obtained a copy of Personnel Report (LIC 500), and Resident Roster (LIC 9020). The home is located in a residential area, the one-story facility includes a living room, dining room, kitchen, laundry section, 4 client bedrooms, 1 bathroom for clients, 1 bathroom for staff and visitors, an unattached garage and indoor/outdoor activity areas.
