Facility Evaluation Report
Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit. LPA was met by Jaqueline Sanchez, DSP II and explained the purpose of today's visit. Shortly after, Maria Carlos, Administrator and Milagros Navas, Care staff arrived and assisted LPA with the inspection. The facility is approved for (6) ambulatory developmentally disabled adults ages 18 through 59 years old. The facility is vendorized by Frank D. Lanterman. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Staff are adhering to infection control requirements. The facility has submitted a COVID-19 Mitigation Plan and the Infection Control Plan.
Operational Requirements: The Infection Control Plan has been added to the Plan of Operation. A fire clearance is in place. Fire and earthquake drills are conducted on a regular basis. Surety bond is in place, copy of the current insurance certificate will be emailed to LPA. The facility provides a shaded and furnished outdoor activity area.
Physical Plant & Environment Safety: This facility is a single story home consists of kitchen, dining room, living room with covered fireplace, (3) client bedrooms, office area, (2) bathrooms, laundry area, and a detached garage. Currently, there are (5) clients residing in the home. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Smoke alarms and carbon monoxide were tested and operable. Laundry area is next to the kitchen. LPA observed (2) fire extinguishers, in the laundry area and the hallway, and both were serviced on 09/06/2024. Knives, cleaning solutions, and disinfectants are locked in a cabinet and inaccessible to clients. There are no firearms or weapons stored at the facility. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Exit doors are free of any ob struction and there are no pools or large bodies of water. Backyard was inspected and has a shaded area and sitting area. LPA observed that the area outside the detached garage is filled with miscellaneous trash items. *****Refer to LIC 809C for the continuation of this report.*****
