Facility Evaluation Report

On 7/16/26, Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced Annual/Required inspection to Jaswell Home Care. Upon arrival LPA was greeted by the Direct Support Professional (DSP) Virgina Bajao. The Administrator Jasfer Cruz arrived at 2:15 pm. LPA explained the reason for the visit.

This home is licensed to serve ages 18 through 59. Approved for 2 ambulatory and 2 non-ambulatory. Only bedrooms #3 and #4 approved for non-ambulatory. Clients receive services through the South Central Regional Center. Facility is located in a residential neighborhood of Paramount, CA.

There were four (4) clients in care during the time of this visit. The last emergency disaster/fire drill was conducted on 5/7/2026. The Administrator Certificate expired on 3/07/2027. During today's visit LPA inspected the physical plant inside and outside, reviewed the food supply, tested the smoke/carbon monoxide detectors, reviewed three (3) staff files, four (4) client files, medications, and medication administration records for four (4) clients and P&I.
