The inspector’s account

Licensing Program Analyst (LPA) Elena Mallett arrived at the facility to conduct the Annual Visit. LPA was met by Staff in Charge David Berry and explained the purpose of the visit. Licensee/Program Director/Administrator Maxine Hartwell was contacted by phone.

Staff in charge Berry explained to LPA that facility was not currently operating with clients and has a census of 0. The facility is in the process of getting vendorized by South Central Los Angeles Regional Center. Facility provided a copy of email sent to Licensing 03/23/2026 detailing the situation.

The Day Program is licensed to serve 30 adults 18 and over of which 6 can be non-ambulatory. The facility consists of a main room, one relaxation room, four restrooms, a kitchen and one activity room and area for storing clients' belongings. There is a small patio out in the front of the building that clients can sit at a shaded table. The table and umbrella was observed to be storage and will be put up again once clients are at facility per Administrator The facility is sanitary and all hallways and common areas were observed to be free of debris and obstruction

Infection Control: Facility has an infection control plan that is followed.

Physical Plant & Environment Safety: LPA toured facility. The facility is well maintained and halls and passage ways free from obstruction and debris. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. A locked cabinet to store cleaning solutions, toxins, and hazardous items was observed and is inaccessible to clients. Smoke detectors and carbon monoxide detectors are operable A fully charged fire extinguisher was present. The restrooms were all observed to have grab bars. Three of the four bathrooms had hot water that measured within Title 22 Regulations of (105F-120F). The water in Bathroom 3 did not. A deficiency was cited. See 809-D
