Facility Evaluation Report
Licensing Program Analyst (LPA) Mayra Cota, conducted a pre-licensing inspection on 5/8/25. LPA met with Wilson Smith, Assistant Administrator who assisted with the visit. LIcensee, Maxine Hartwell, arrived thereafter.
The fire clearance is granted for 24 ambulatory and six (6) non-ambulatory clients; total capacity 30. The hours of operation will be 7:30 a.m. - 3:30 p.m., Monday through Friday. The single-story building is located in a residential area of Paramount.
LPA inspected the prospective day program using the Compliance and Regulatory Enforcement (CARE) tool and completed the Component III portion of the pre-licensing process with Licensee and Assistant Administrator.
The facility consists of the following: main activity room (Dept. B), resting room (Dept. A), four (4) gender neutral restrooms, kitchen, and a small group activity room, (Dept. C).
The following was observed during today's inspection:
The building and grounds are free of hazards and both inside and outside of the facility are clean and in good repair. Activity rooms have sufficient tables and chairs for client activity engagement and furniture is in good repair. The facility has one fire extinguisher which was last inspected on 2/25/25 and was observed charged and operable. Two carbon monoxide detectors were tested and working properly. Facility is also equipped with two (2) interconnected, combination smoke and carbon monoxide detectors and two (2) stand-alone smoke detectors, which were also tested and working properly.
