Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit. The purpose of the visit was to confirm there are no residents living in the facility. On 9/19/2024, LPA attempted to conduct an annual visit. However, upon arrival, LPA observed the gate at the driveway entrance was locked. LPA observed there were no cars on the property and there was no indication that residents were living in the facility. LPA called the Licensee Narek Hovannesyan and left a voicemail message.
LPA received an email on 9/25/2024 from the Licensee stating there are no residents living in the facility. The Licensee also stated they are out of the country but will be returning October, 2024. The Licensee stated they will contact LPA Spaeth once they have returned. However, LPA did not receive any communication from the Licensee. LPA left a message for the Licensee on 4/15/2025 and LPA received a phone call from the Licensee on 4/16/2025. LPA stated the purpose of the call was to remind the Licensee an annual inspection is required based upon Title 22 Regulations.
As of today, LPA Spaeth and the Licensee toured the facility at 11:00 am until 11:05 am. LPA observed there were no residents living at the facility.
Exit interview conducted and a copy of the report was given.
