Complaint Investigation Report
Regarding the allegation: Staff left resident unattended. It is alleged a client was left alone by their one-on-one staff member. On 7/03/2025, C1 was escorted to an
appointment by a staff member (S1). C1 asked to visit their family member after the appointment and C1 was escorted by S1 to visit the family member. During the investigation, LPA Spaeth determined C1 was taken to a medical appointment by S1. C1 also requested to visit a family member after the appointment and S1 agreed to the request. S1 stated the visit was for a couple of minutes; however, C1 refused to return to the facility. During interviews, C1 confirmed S1 escorted C1 to their doctor’s appointment and then to visit a family member. C1 confirmed they were upset when it was time to leave and stated they did not want to return to the facility but wanted to stay with their family member.
Based upon LPA’s interviews, the allegation is unsubstantiated .
