Complaint Investigation Report

Regarding the allegation: Staff handled resident in a rough manner causing injury. It is alleged a staff member shoved a client against a door which caused the client pain. During interviews with staff, S1-S3 denied the allegation. During interviews with clients, one out of four clients confirmed this occurred and one client confirmed this did not occur. LPA Spaeth attempted to obtain the urgent care after summary, but C1 refused.

Based on interviews, there is insufficient evidence to verify the allegation. The allegation is hereby unsubstantiated.

Exit interview conducted and a copy of the report was given.
