Facility Evaluation Report

Licensing Program Analyst (LPA) Tuesday Cabiness attempted to conduct an annual required inspection. LPA approached the front gate property and was not able to enter, which was locked. LPA contacted the Administrator Akiko Sadamoto, who informed LPA the facility has ceased operation and the property was sold to a new owner. Administrator also informed LPA, she notified the department and Licensing in October 2025. LPA requested for her to send a copy of the letter and she will officially close the facility internally.

No further action is required at this time, and the facility will be closed. LPA also received the current mailing address to send the closure letter to the Licensee.
