Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley visited the facility in reference to a situation that happened on 05/22/25, with resident #1. The resident arrived to the facility on 04/01/25. The facility indicated that the resident became upset because the administrator closed the door to the office while she was speaking to another employee. They opened the door and she went into her bedroom and got some beads that she made. They say she is really crafty and likes to paint and make things out of beads and yarn She was in the living room and said I don’t want to live anymore with the beads around her neck and she began to pull the beads, they tightened, with loops on the end, the staff went to get the scissors and began cutting the beads from her neck. LPA requested a copy of the residents ID page, Mar log, Physicians report, IPP. The administrator will fax the documents to Attn: Nicol Wesley, 323 980 4912 by tomorrow 06/04/2025 at 5pm.

The facility did not report the incident to licensing and will be cited according to the California code of regulations title 22, division 6 chapter 1. Appeal rights given.
