The inspector’s account

LPA conducted interviews with two clients and seven staff members, all of whom denied the allegation. S1 also denied the allegation and stated that the accusations were false. S1 further stated that C1 is alert and would have reported any inappropriate behavior if such incidents had occurred. The Administrator stated that the facility staffs seven employees per shift and that any allegation or incident involving sexual abuse would have been self-reported to the appropriate agencies. The LPA reviewed the facility’s incident report records and did not observe any documented or reported incidents related to sexual abuse. Based on interviews conducted and records reviewed, there is insufficient evidence to support the allegation. Therefore, the allegation is deemed Unsubstantiated at this time

Regarding the allegation: Staff hit resident.

It is alleged that Staff#2 (S2) hit Client#2 (C2) in the head. LPA interviewed S2, who denied the allegation and stated that all client rights are respected and that physical abuse of clients is prohibited under any circumstances. Interviews conducted with six staff members did not confirm the allegation. LPA attempted to interview C2; however, C2 is non-verbal and unable to provide a statement. The Administrator stated that the facility has a zero-tolerance policy regarding any form of violence or abuse toward clients. Based on interviews conducted, there is insufficient evidence to support the allegation. Therefore, the allegation is deemed Unsubstantiated at this time.

Regarding the allegation: Staff does not provide adequate food service for residents.

It is alleged that clients are not properly fed, resulting in Client #1(C1) developing diabetes. LPA interviewed the Administrator, who provided medical documentation indicating that C1 has not developed diabetes. The LPA conducted a physical plant tour of the facility’s kitchen and observed the following: weekly menus, food and liquid consistency reminder postings, diet/eating matrices, and adequate supplies of both perishable and non-perishable food items.Interviews conducted with two clients and seven staff members denied the allegation. Additionally, five staff members confirmed that the facility follows all prescribed dietary restrictions and dietary care plans for clients. LPA reviewed C1’s After Visit Summary dated 04/29/2026 and Individual Program Plan (IPP) dated 04/17/2025. Neither document indicated that C1 had developed diabetes. Based on interviews conducted, observations made, and records reviewed, there is insufficient evidence to support the allegation. Therefore, the allegation is deemed Unsubstantiated at this time.
