Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted a Case Management-Incident visit. LPA met with Administrator Jonathan Watts and explained the reason for the visit. On October 11, 2025, Community Care Licensing (CCL) received a Special Incident Report (SIR) from facility Administrator Jonathan Watts, reporting that Client #1 (C1) was AWOL from the facility. On October 9, 2025, the facility Administrator was informed that Client#1 (C1) had eloped from the facility while Staff#1 (S1) was using the restroom. According to the Administrator, C1 had been AWOL for a day and had returned to the facility on October 10, 2025. Interview with the Administrator revealed that on the day of the incident, there was only one staff member present (S1) with C1. LPA reviewed with the Administrator the previous Plan of Corrections given regarding the same matter. Based on information obtained, facility staff were not competent to provide the necessary services to meet clients' needs.
Exit interview conducted, civil penalty for repeated violation is issued, appeal rights given, and a copy of this report delivered.
