Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted a Case Management-Incident visit. LPA met with Administrator Jonathan Watts and explained the reason for the visit. On September 8, 2025, Community Care Licensing (CCL) received a Special Incident Report (SIR) from facility Administrator Jonathan Watts, reporting that Client #1 (C1) was AWOL from the facility. On September 7, 2025, the facility Administrator was informed that Client#1 (C1) had eloped from the facility while Staff#1 (S1) was using the restroom. According to the Administrator, C1 had been AWOL for the night of September 7, 2025, and had returned to the facility on the next day, September 8, 2025. Interview with the Administrator revealed that on the day of the incident, there were 4 staff members present. However, there was miscommunication between facility staff that resulted to C1's AWOL. Based on information obtained facility staff was not competent to provide the services necessary to meet clients needs.

Exit interview conducted, civil penalty for repeated violation is issued, appeal rights given, and a copy of this report delivered.
