Facility Evaluation Report

In conjunction with the Annual Required visit and inspection of the facility, Licensing Program Analyst (LPA) Mariana Agban conducted a Case Management-Incident visit. The LPA met with Administrator Jonathan Watts and explained the reason for the visit. On April 26, 2025, Community Care Licensing (CCL) received a Report of Suspected Dependent Adult/Elder Abuse (SOC 341) from facility Administrator Jonathan Watts. On 04/22/25, the facility Administrator was informed that Client#1 (C1) had eloped from the facility due to Staff#1 (S1) neglect. According to the Administrator, C1 had been AWOL from 04/22/25 to 04/25/25. The Administrator stated that Local Law Enforcement was contacted and reported the AWOL. The LPA interviewed two staff members, the facility's BCMA and Client 1 (C1), regarding the incident. The interviews confirmed that there was only one staff member (S1) at the facility. Thus, there was an insufficient number of staff present at the facility. The LPA obtained copies of the police report and Special Incident Report (SIR).
