Facility Evaluation Report

An Informal Conference was conducted today in the Woodland Hills Adult and Senior Care Regional office. The purpose of this meeting was to discuss recent issues of non-compliance.

Present at today's meeting is the following:

· Eva Miller, Licensing Program Manager (LPM)

· Mariana Agban, Licensing Program Analyst (LPA)

The informal conference process was explained to the Administrator. The Administrator was also informed that this Informal Conference is a part of the administrative action process. Further citations may result in a Non-Compliance Conference, which could lead to a referral to the Department's Legal Division for possible license revocation or other administrative actions.

BRIEF HISTORY : The department had received multiple complaints in 2023 and 2024 about Client#1 (C1) and the facility staff's failure to provide the required care and supervision. This had resulted in multiple substantiated allegations and non-compliance with the facility staff. Substantiated allegations were Staff did not adequately supervise residents resulting in a resident being physically assaulted by another resident while in care, Licensee does not ensure that the facility has a certified administrator, Staff are not being properly trained, Staff are operating out of ratio, Staff are not providing adequate care and supervision to a client, and Staff do not treat resident(s) with dignity and respect.
