Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted an unannounced continuation of the annual visit. The LPA met with Administrator Jonathan Watts and explained the reason for the visit. During today's visit, the LPA reviewed files for two (2) clients to ensure compliance with licensing forms. LPA also reviewed files for four (4) staff to ensure that forms and training are current and comply with licensing requirements. LPA with the assistance of the Administrator reviewed cash resources for Client#1 and Client#2. Logs and cash resources were consistent. Client#3 is independent with managing cash resources. Finally, LPA conducted a review of medication records. Medication records are maintained electronically. Medication and Medication Records were consisted.

Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of the Report Issued.
