This is an official report of an unannounced visit/investigation of a complaint received in our office on

05/28/2024 and conducted by Evaluator Mariana Agban

COMPLAINT CONTROL NUMBER: 31-AS-20240528120602

Licensing Program Analyst (LPA) Mariana Agban conducted subsequent complaint visit to investigate the above allegation. LPA arrived at the facility and was granted access. Administrator Jonathan Watts was present at the facility and explained the reason for the visit.

Allegation: Staff are not being properly trained

The complainant alleged that staff are not being properly trained. Interview with Administrator revealed that he was hired on June of 2024 and currently the facility doesn't have a copy of staff training for the timeline of May of 2024. Administrator provided staff training from July of 2024 until present. LPA advised Administrator that all records shall be maintained at the facility site.
