Complaint Investigation Report

Allegation: Facility does not provide client's access to community activities

It was alleged that staff do not take clients out on weekend outings. Interviews with three (3) staff members confirmed that staff take out clients on outings on the weekends. LPA obtained records of facility outings, which confirmed that clients are going on outings on weekdays and weekends. Based on the information obtained, the allegation is deemed Unsubstantiated at this time.
