Facility Evaluation Report
On 09/02/25 , at 9:30am , Licensing Program Analyst (LPA) Gina Saucedo arrived at the above facility to conduct an announced case management visit with Debra Ingram, Office Manager/HR Specialist for the purpose of closing the facility and cease operation. An application was submitted to Community Care Licensing Division-CCLD on 09/16/2024 for change of location. LPA conducted a walk-through of the above facility to ensure there were no participants and to officially close the facility. LPA observed no evidence that the facility is being operated as an ADULT DAY PROGRAM; no deficiencies cited during this visit.
LPA Saucedo will submit the file for closure upon returning to the Woodland Hills South Regional Office.
The report was signed during the visit, and a copy of the report will be given to the Office Manager/HR Specialist.
