Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an Annual Required visit and inspection of the facility. LPA met with staff and explained the reason for the visit. Administrator Christian Olano was contacted and arrived shortly after. At approximately 09:30 am, with the assistance of staff, LPA took a tour of the physical plant. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility, properly stored. Required postings were observed. Laundry area: is located next to the kitchen. LPA observed the washer and dryer to be in good repair. There were multiple cabinets where the laundry toxins, sharp objects, and clients' medications were stored. LPA tested the fire alarms, and they were functional. LPA observed the fire extinguisher that is located in the kitchen. The purchase date is October 10, 2025. Bedrooms: There were three (3) bedrooms designated for clients' use. Two (2) bedrooms are designated for private use, and one (1) bedroom is shared. All bedrooms, in use by clients, were properly furnished with appropriate beddings and linens with sufficient lighting. There is a carbon monoxide detector that functions properly installed in the hallway between resident rooms. Bathrooms: There is one (1) bathroom designated for clients' use. The bathroom was properly supplied and had functional fixtures. Hot water temperature was measured from the bathroom sink at 108.3 degrees Fahrenheit. No cleaning supplies or hazardous items was present during the inspection.
Common Areas: These included the living room and dining area. The common areas were properly furnished. There is a fireplace that is properly screened. The fireplace is non-functional. No key to the fireplace, and fireplace tools were not present. The dining room table is large enough to seat the capacity of the facility. Seating, such as couches where in good repair and has the capacity of the facility.
