Facility Evaluation Report
Licensing Program Analyst (LPA) Sakinah Madyun conducted an unannounced annual inspection visit at the facility using the CARE inspection tool. LPA met with Case Manager Monique Davis and explained the reason of the visit.
The facility is licensed to serve (60) Developmentally Disabled Adults, 40 Ambulatory and 20 Non-Ambulatory, age range 18 and over , which operates Monday through Friday from 8:00 AM to 2:00 PM. A tour of the business complex facility began at approximately 9:05am, that consists of a front desk/lobby area, three (3) staff offices, four (4) activity rooms, two (2) relaxation/one-to-one rooms, a conference room, a computer room, a vocational room, a kitchen, two (2) janitorial/storage rooms, and four (4) restrooms.
LPA toured the facility with Case Manager Monique Davis and observed the following:
The facility is in good repair inside and outside. Each activity area was observed clean with sufficient work and sitting furniture and within a ratio ranging of one (1) staff per one (1) participant, one (1) staff per two (2) participants, and one (1) staff per three (3) participants. All other areas were observed clean and free of debris, sharps, or hazards. Kitchen area is clean, cabinets are securely locked and used to store cooking activity supplies and utensils, and special request or personal snacks items. Participants refrigerator was observed to be clean. Bathrooms were observed to be clean and in good repair. Water temperature was tested in each bathrooms' sink and kitchens' sink and tested between 112.0-119.3 degrees Fahrenheit., which is within the required 105-120 degrees Fahrenheit. First Aid kit was observed and located in the staff office attached on he wall with a current manual. (3) Fire Extinguishers were observed to be full and last inspected 05/22/25. The outdoor area was observed with a shaded patio area. Smoke/Carbon monoxide detectors and a fire sprinkler system were observed throughout the facility. LPA was unable to test smoke alarms, the Regional Manager will provide proof of the last inspection and forward to the department the following week. Continued LIC 809C
