Facility Evaluation Report
At 12:30pm, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. LPA met with the Administrator and explained the reason for the visit.
A tour of the physical plant was initiated at 12:40pm and the following was observed:
This is a Short-Term Crisis Residential Program, approved for capacity of six (6) Ambulatory clients. LPA was informed that the facility has awake staff at night.
Kitchen: The kitchen area is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6) clients. Sharp knives/objects are kept in the kitchen closed and observed to be locked and inaccessible to clients in care.
Bedrooms: There are four (4) bedrooms designated for clients’ use, of which one (1) private bedroom is downstairs, and three (3) bedrooms upstairs (1-private and 2-shared) . All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space.
Bathrooms: Facility has five (5) and half (½) bathrooms. Bathrooms have soap and paper towels. Extra towels and linens were readily available. The hot water delivered in the client bathrooms measured at 114.0°F.
Common Areas: The facility maintains a comfortable temperature of 71°F. LPA observed all common areas are furnished with adequate furniture to accommodate a maximum capacity of six (6) clients. There is a
